Scouts: Class
II Medical Form (signed by a doctor) -- it's good for 3 years (cannot be
dated more than 3 years from the last day of camp), but must be accompanied
by a Class
I Medical Form (signed by parent) if the Class II Form is more than 1
year old.
Adult Leaders Under 40: same as for
the Scouts.
Adult Leaders 40 & Older: Class
III Medical Form (signed by a doctor), and it must be done yearly.
"Visiting Leader" -- defined as an
Adult that spends less than 72 hours at camp: Class
I Medical Form (signed by the Adult). However, the Visiting Leader
cannot swim at camp (if you want to swim, you have to follow the "Adult
Leader" guidelines).
"Visitors" -- defined as an Adult that
drops off or picks up a Scout: No Health History Form required.
However, visitors are not allowed to remain during camp, except during
"family time" (if you want to stay at camp, you are considered an
Adult Leader and must fill out the Health History Form).
